Make sure that the review manager has a chance to review the completed Employee Appraisal Form.
If the Employee Appraisal Form is being used to assess the employee’s potential for promotion and the review manager disagrees with any of the reporting manager’s points, these should be recorded on the Form along with the reason for the disagreement. Once the review manager has reviewed the Employee Appraisal Form, the employee should have a chance to review the document.
A follow-up meeting should then be scheduled between the employee and the reporting manager to discuss the outcome of the evaluation. It is recommended that you hold the next meeting within a week after the first performance review meeting.

Signing in print
Print two copies of the Employee Appraisal Form.
The reporting manager should sign and date both copies.
The review manager should sign and date both copies.
The employee should sign and date both copies.
One copy of the signed and dated Employee Appraisal Form should be given to the employee. The employee should then keep their copy of the completed and signed document.
The employer’s completed and signed Employee Appraisal Form should be kept as a record on the employee’s file and maintained as confidential.
