How do I change my name in Florida?
When a name change in Florida is part of a marriage, divorce or adoption, the request is processed as part of that request.
Otherwise, changing your name in Florida is a different process. First create your Petition for Change of Name and get it notarized. Once it is notarized, file it with your county's clerk of the circuit court. When filing you will be required to submit fingerprints and agree to a statewide and nationwide criminal background check. Once you have received the results of the criminal background check, you need a court hearing to formally request your name change – some counties automatically assign court dates while others require you to request a date so consult with your circuit clerk regarding the process in your county. You may be required to attend the hearing. If so bring a valid ID and all supporting paperwork to prove that you meet the requirements of a name change in Florida.
