What is a Travel Consent Form?

When to use a Travel Consent Form:
- Your child will be traveling alone within the US or internationally.
- Your child will be traveling outside the country with their other parent.
- Your child will be traveling within the US or internationally with another adult.
- You will be traveling with minors, as a coach or other chaperone, and need forms for parents.

Instructions for Travel Consent Form
If one legal parent is traveling by air or crossing a border with a minor child, this form should be signed by the legal parent that is not traveling with the child.
If a minor child is traveling by air or crossing a border with someone other than either legal parent, BOTH legal parents should sign this form.
If one or both legal parents are deceased, a certified death certificate should be attached to this form. If one of the legal parent's whereabouts is unknown, a separate affidavit stating such is suggested.
"Legal parents" includes individuals who are the legal guardians of the child.

How do I write a letter of permission for my child to travel?
You can quickly and easily draft a letter of permission for your child to travel with this template. At a minimum, the permission letter should provide your child's name and date of birth, the dates of trip, the name of the adult companion, the specific travel destinations, and the reason for travel. You should also make sure to include both parents' full names, signatures, and phone numbers.

If your child is traveling within the United States, notarization is typically not required. International travel requirements vary by country.
