What is an Advance Directive Revocation?

An Advance Directive Revocation is a document that officially cancels an existing Advance Health Care Directive. While many states allow Advance Directives to be revoked at any time and in any manner, it's best to provide a written, signed, and dated revocation to the agent (if any) and the appropriate healthcare providers.

After making your Advance Directive Revocation, it's crucial to notify your agent and physicians of the revocation as soon as possible. Otherwise, in many states, the document may be considered ineffective until you have done so. Additionally, unlike other documents that require a witness or notary, a revocation does not need to be notarized or witnessed. You can simply sign the document and use it to inform your agent and healthcare providers.
