What a Leave of Absence Form Provides
As an employer, a leave of absence will allow you to retain the employee's status within your business. This is to say your employee will remain obligated to work for you and will retain their position when they return to work.
What is Included
You cancreate a free leave of absence form online that will outline the terms of the leave. This will include the following information:
- Reason for Leave: The reason for the employee's leave must be listed on the form.
- Duration: The commencement and the end of the leave granted. This will include the exact dates of leaving and returning to work.
- Compensation: Employees may or may not be compensated during their absence. Unpaid leaves are quite common, but this is at the discretion of the employer. At this time, you may also retain your health coverage in some cases.
As an owner of a business, you must set forth an application process wherein any employees can fill out the leave of absence form and request their leave. A policy should also be drafted that dictates when leave will be granted and under what circumstances. There are some forms of leave that are mandatory under the Family and Medical Leave Act and USERRA.
